Registration
There are two weeks of the Streamline Swim Clinic. The first week costs $440 and includes lunch at the Dana Hall Dining Hall. The second week costst $400 but does not include lunch. The Clinic also offers several discounts to those who are eligible:
Early Application – Mail the registration forms before May 1st, and you will receive a $50 discount.
Family Discount – Parents who wish to sign up more than one of their children will be offered a discount of $25 for each of two or more children.
A non-refundable deposit of $75.00 per child per week is required with the Registration Form.

Registration Forms:Mail your registration form before May 1st and automatically receive a $50 discount. The complete health forms including a physical and vaccination must be mailed one month prior to camp start date.

Streamline Swim Clinic | Health Forms:The first two pages can be supplemented with a physical and immunization records from your child's doctor. However, if you do so, please fill out the personal and parent information on the first page.
